Deliverables:- 

Ensure the implementation of the project objective at 3 sites, namely (Tshilidzini, Donald Fraser and Siloam Hospitals)

Strengthen existing and establish new strategic partnerships governed by service level agreements.  

Conduct home visits to all clients on PEP to support adherence

When indicated, provide referral and conduct follow up support to approved service providers.         

Collect client data and maintain accurate records

Provide value added care in accordance to the list of services
